Do I need to prepare for a session? What should I bring?

Before your appointment: ​

Please have your prescription for Physical Therapy, if you have one, and your insurance information available when you contact us. We will verify your insurance benefits and provide you with an estimate of your coverage for services. If you don't have a prescription, fear not! In some cases, you are able to see us directly without an order. We also have options for those that do not have insurance. In these cases, please contact us for more details. 

Preparing for your first appointment: ​

Please arrive 10 minutes early in order to complete necessary paperwork so that we can allow maximal time for your session. Please bring: 

  • A prescription (if applicable) for Physical Therapy from your referring medical provider. 

  • Insurance card. 

  • Photo ID.

  • Comfortable clothing that you can move in so we can fully assess your condition. 

  • Your completed PT intake forms, symptom questionnaire and policy form

What can I expect at a Physical Therapy session? 

All visits are scheduled with a Doctor of Physical Therapy. We provide one-on-one evaluations and develop an individualized plan to achieve the specific results you are seeking. All sessions following typically last around 60 minutes. 

How many sessions will I need? 

Each plan of care varies from person to person based on your specific needs, current condition, and goals. Your Doctor of Physical Therapy will work with you at the first visit to establish a specific plan to address your needs and goals. This plan can be modified throughout the course of your treatment to make sure you achieve optimum results. 

Where should I park?

Parking is readily available at multiple close-by locations. Below is a map of Milwaukee city surface lots at $1.50/hour (prices subject to change).
